EvergreenHealth Receives Leapfrog Spring 2024 "A" Safety Grade

11 Consecutive "A" Grades 5.5 years in a row (2019–May 2024)
Once again, EvergreenHealth has been recognized by The Leapfrog Group with an "A" safety grade, acknowledging our exceptional care and unwavering commitment to safety, every day at every point of service across the organization.
This recognition by The Leapfrog Group – our 11th consecutive – rigorously measures and compares hospital safety. Leapfrog Hospital Safety Grades are assigned to nearly 3,000 general acute-care hospitals across the nation twice annually. The Safety Grade uses multiple sources, including more than 30 national performance measures from the Centers for Medicare & Medicaid Services (CMS) and the Leapfrog Hospital Survey. Taken together, these performance measures produce a single letter grade representing a hospital's overall performance in keeping patients safe from preventable harm and medical errors.
One of the best examples of how we consistently deliver high quality, effective, and safe care is our daily Safety and Service Huddle. What began as a forum to rapidly share real-time information and address issues in the hospital has evolved into a critical organization-wide quality tool. This meeting helps raise situational awareness related to patient flow, bed census and safety events – many of which are solved in real-time by the more than 100 attendees. A regular cadence of reports from quality, patient experience, supply chain and others deliver additional vital information in a concise and impactful format. This meeting is central to the way that we drive toward our goal of performing as a high reliability organization, ensuring that we deliver the care for which we are known and to which we hold ourselves accountable.
Our latest recognition from Leapfrog is another milestone in our year-over-year safety record, which includes:
- Recognition by Healthgrades earlier this year for the fourth year in a row as one of America's 50 best hospitals (2021-2024), placing us in the top 1% of nearly 4,500 hospitals assessed nationwide for consistently superior clinical performance
- Recognition by CMS for eight years in a row with a CMS 5-Star Rating, which summarizes a variety of measures across five areas of quality into a single star rating for each hospital. The five measure groups include: mortality, safety of care, readmission, patients experience and timely and effective care.
